The Pathways telehealth nurse program was developed by the Centre for Community-Driven Research (who developed and host Liver Cancer Support Australia). Pathways is a free service that any liver cancer patient can access, regardless of their location.
Families affected by liver cancer have accessed the service and we are calling on the community to donate and ensure the continuation of a dedicated liver cancer Pathways Nurse service.
Pathways nurses conduct a holistic consultation to understand everything that is happening with the patient in front of them. It’s a model of care that helps people navigate the complex health system and access all that is available to them. The Pathways nurse then works with the patient to develop a care plan and offers support and guidance along the way.
